• Student/Senior    $20.00 per adjustment  

                    *Aditional family members will be $20.00 per person

• Progressive Scan  $25.00

Payments are due at the time of service. We are not a networked provider for any insurance company. This is for your benefit, as to keep our fees low.  If you have insurance that covers any of our services, we have an external billing department that will be happy to bill your insurance for you. We do NOT accept assignment from any insurance carrier.  This means that reimbursement checks get sent to YOU.  Our external billing company works hard to get your reimbursement to you as this is the only way they are compensated by Pivotal Wellness. This is a service that we provide for you.  This is to safeguard you and the quality of care we are dedicated to providing all of our practice members, with or without insurance.
